Anamnesis - Taking a patient’s medical history is a key issue for medical diagnostics

When first meeting, it is key to us as physicians getting to know our patients’ medical history by asking about their lifestyle, habits and recent troubles or health disorders. This will help define the manner and scope of the following diagnostic steps to be taken. The notion of a patient’s “medical history”, the anamnesis, alludes to remembering and gathering the essential information about a patient’s life to date.

To facilitate taking your medical history, we have prepared a patient questionnaire that you can already print at home and then easily fill in. Please make sure to have the completed questionnaire with you when coming to see us for the first time.
